I understand that telehealth involves the use of electronic communications to enable healthcare providers to evaluate, diagnose, and treat patients without an in-person visit.
I acknowledge:
The provider may not perform a physical examination.
The provider relies on the information I submit and a video call.
There are limitations and risks associated with telehealth.
Telehealth services are provided in accordance with applicable state and federal laws and regulations.
4. Medical Information and Responsibility
I certify that:
I have fully and accurately disclosed my medical history.
I will update my information as needed and I will seek in-person care when appropriate.
I understand that incomplete or inaccurate information may impact the quality of care I receive or cause reason for denial of service.
5. Prescriptions and Pharmacy
I understand that:
If prescribed, medications are issued solely at the discretion of the licensed healthcare provider and may be fulfilled by a licensed pharmacy, which may be located in a different state in accordance with applicable laws. Pharmacies are independent entities responsible for dispensing medications.
I agree to use any prescribed medication only as directed and for my personal use.
6. Relationship of the Parties
I understand and agree that:
Healthcare providers are independent contractors and not employees of this website.
Pharmacies are independent entities.
This website does not control or influence medical or pharmacy decisions.
7. Risks and No Guarantee
I understand that all medical treatments carry risks and that results may vary. No guarantees have been made regarding the effectiveness of any treatment or medication.
8. Limitation of Liability
To the fullest extent permitted by law, this website, it's owner(s), affiliates are not liable for any indirect, incidental, or consequential damages arising from the use of services, medical decisions, or prescription fulfillment, which are the responsibility of independent providers and pharmacies.
9. Dispute Resolution
In the event of any dispute, claim, or controversy arising out of or relating to the use of this website or services:
The parties agree to first attempt to resolve the dispute through good-faith mediation in the State of Florida. If the dispute is not resolved within thirty (30) days, it shall be resolved by binding arbitration administered by the American Arbitration Association in accordance with its applicable rules
All disputes shall be resolved by binding arbitration administered in the State of Florida. Arbitration may be conducted in person or remotely by video conference, as mutually agreed or determined by the arbitrator.
The arbitration decision shall be final and binding, and judgment may be entered in any court of competent jurisdiction. This agreement shall be governed by the laws of the State of Florida, without regard to conflict of law principles.

I understand that my personal and medical information will be collected, used, and stored in accordance with applicable privacy laws and used for purposes of treatment, payment, and healthcare operations.
